Live Well is a speaker series with the purpose of addressing personal, spiritual, and financial well-being for SLU employees. Speakers include SLUCare physicians, faculty, staff and outside partners who volunteer to share their expertise. New employees and spouses, if covered under medical insurance, have 31 days from their date of benefit eligibility to comply with the two requirements of the Vitality Wellness Plan in order to receive a discount on medical premiums.
If you have had the required tests done within 90 days of your eligibility, have your physician complete the Vitality Check Form and fax/email it to Vitality to meet this requirement.
All full-time employees, regardless of coverage under the medical plan, are eligible to participate in the wellness program. Spouses listed in Banner Self-Service will be auto-enrolled in the Vitality program so that they, too, can reap the rewards of the program.
You will first need to take the Vitality check bio-metric screening. Throughout the year, new employee screenings may be scheduled through MyCare physicians in Family and Community Medicine at 314-977-3500. Employees may also visit their personal physicians. During Vitality's open enrollment in October and November, this screening may be scheduled through SSM Health Saint Louis University Hospital Outpatient Services on or after September 19 at 314-685-3398 or on their website.
After your screening, you will need to complete the Vitality Health Review (VHR) on Vitality's website.
