Schedule Vitality Checks for 2017

Vitality Checks, which are required for Saint Louis University employees and covered spouses to receive a discount on health insurance in 2017, again will be offered by SSM SLU Hospital Outpatient Services.

Appointments with SLUH Outpatient Services will be conducted at Simon Recreation Center at 3639 Laclede Ave. from Oct. 1 through 28, and at the SLUH Outpatient Services clinic at 3655 Vista Ave. from Oct. 31 through Nov. 18.

These free screenings with SLUH Outpatient Services will be by appointment only.

As in the past, employees are also welcome to go to their personal physicians; however, additional charges may apply.

Employees are reminded that in order to be eligible for the Wellness medical premium discounts for 2017, they and covered spouses must complete both the Vitality Check biometric screening, as well as the Vitality Health Review (VHR) through powerofvitality.com between Oct. 1, and Friday, Nov. 18. Vitality Checks and VHRs completed before or after these dates will not be considered for the 2017 Wellness premium discounts.
