Office of Risk Management and Insurance Transitions Forms to Digital Format

In an effort to streamline processes and enhance security, the Office of Risk Management and Insurance has announced that incident reporting forms and certificate of insurance request forms are now available digitally.
Transitioning to digital forms provides several benefits. First, users can conveniently complete and submit forms online from anywhere at any time, eliminating the physical paperwork and in-person submissions. Additionally, the digital format improves the security of sensitive information.
User can access the digital forms through the Risk Management and Insurance website. Or, if the user knows the name of the form they need, they can simply navigate to the “Ask SLU” icon within the SLU apps, click on “Ask SLU”, search on the form name, and select the form from the list.
Risk Management encourages everyone to utilize the digital version for the following forms:
- Certificate of Insurance Request for Third Party
- Certificate of Insurance Request for Evidence of Medical Professional Liability
- Employee Report of Injury
- Non-Employee Injury Form
- Damage and/or Theft Reporting Form
- Medical Adverse Incident Reporting Form
- Vehicle Accident Reporting Form (Involving a university-owned vehicle)
